Twenty years of research into CFS have uncovered many biologic findings about the way this illness affects the body. Here's a "print-and-go" handout listing 10 key discoveries, as described by Anthony Komaroff, MD, a professor of medicine at Harvard Medical School and a widely respected authority on CFS.

1. Chronic fatigue syndrome is not a form of
depression, and many patients with CFS have
no diagnosable psychiatric disorder. As with
most chronic illnesses, some CFS patients
become depressed because of the impact of
the illness on their lives, but most studies find
that the majority haven’t experienced depression
before the onset of illness.

2.There’s a state of chronic, low-grade immune
activation in CFS. There is evidence of activated
T cells, activation of genes reflecting
immune activation and increased levels of
immune system chemicals called cytokines.

3.There’s substantial evidence of poorly functioning
natural killer (NK) cells—white blood
cells important in fighting viral infections.
Studies differ as to whether there may be
increased numbers of NK cells in CFS
patients.

4. Abnormalities in the white matter of the
brain have been found in CFS patients using
magnetic resonance imaging (MRI) scans.
Typically, these are small (fraction of an inch)
areas just below the cerebral cortex, the
outermost area of the brain hemispheres.
Differences in gray matter volume are also
being observed.

5.Abnormalities in brain metabolism, as indicated
by single photon emission computed
tomography (SPECT) and positron emission
tomography (PET), have been discovered.
Other research suggests there’s something
wrong with energy metabolism and the
oxidative electron transport chain in the
mitochondria of CFS patients.

6. CFS patients experience abnormalities in
multiple neuroendocrine systems in the
brain, particularly depression of the
hypothalamic-pituitary-adrenal (HPA) axis,
but also the hypothalamic-prolactin axis
and hypothamalmic-growth hormone axis.

7. Cognitive impairment is common in CFS
patients. The most frequently documented
abnormalities are difficulty with information
processing, memory and/or attention.

8. Abnormalities of the autonomic nervous
system have been found by numerous independent
researchers. These include a failure
of the body to maintain blood pressure after
a person stands up, abnormal responses of
the heart rate to standing and unusual pooling
of blood in the veins of the legs. Some studies
also find low levels of blood volume.

9. CFS patients have disordered expression of
genes that are important in energy metabolism.
Energy comes from certain natural
chemicals that are processed by enzymes
inside each cell. These enzymes are controlled
by specific genes. Other genomic research is
revealing involvement of genes connected to
HPA axis activity, the sympathetic nervous
system and immune function.

10. There’s evidence of more frequent latent
active infection with various herpesviruses
and enteroviruses. The herpesviruses include
Epstein Barr, HHV-6 and cytomegalovirus.
Other infectious agents, like bacterium that
cause Lyme disease, Ross River virus and
Q fever, can also trigger CFS.

The above summary of CFS research findings was provided by Anthony Komaroff, MD,
a professor of medicine at Harvard Medical School, senior physician at Brigham and Women’s
Hospital in Boston and the editor-in chief of Harvard Health Publications. Dr. Komaroff has an
ongoing research program on chronic fatigue syndrome and has published over 230 research articles and book chapters.

In letteratura non vi è concordanza sulla possibile aumento del numero delle cellule NK nei pazienti con CFS.

4. Utilizzando la Risonanza Magnetica si sono evidenziante anomalie nella sostanza bianca del cervello nei pazienti con CFS. Tipicamente si tratta di aree molto piccole al di sotto della corteccia cerebrale, la parte più’ esterna degli emisferi. Sono state osservate inoltre differenze di volume nella sostanza grigia.

5. Utilizzando la SPECT (tomografia computerizzata a emissione di protone singolo) e la PET (tomografia a emissione di positroni) sono state scoperte anomalie nel metabolismo dell'encefalo. Altre ricerche suggeriscono che nei pazienti con CFS vi sia un disordine nel metabolismo energetico e nella catena di trasporto degli elettroni della fosforilazione ossidativa a livello mitocondriale.

6. Sono state riscontrate nei pazienti con CFS delle anomalie in molteplici sistemi neuroendocrini cerebrali , in particolare minor attivazione dell’asse ipotalamo-ipofisi-surrene (HPA), ma anche dell’asse ipotalamo-prolattina e dell’asse ipotalamo-ormone della crescita.

7. Nei pazienti con CFS è frequente anche un deficit cognitivo. Le anomalie più spesso documentate riguardano le difficoltà nella elaborazione delle informazioni, i disturbi della memoria e dell’attenzione.

8. Numerosi studi indipendenti hanno dimostrato anomalie del Sistema Nervoso Autonomo. Queste includono la difficoltà a mantenere la pressione ematica (sanguigna) passando alla posizione eretta, risposte anomale della frequenza cardiaca in ortostatismo e una alterata quantità di pool ematico nelle vene degli arti inferiori. Alcuni studi dimostrano, inoltre, bassi livelli del volume ematico totale.

9. I pazienti con CFS hanno un’alterazione nell'espressione dei geni che sono importanti per il metabolismo energetico. L’energia deriva da certe sostanze chimiche naturali che vengono metabolizzate (elaborate) dagli enzimi presenti in ogni singola cellula. Questi enzimi sono controllati da geni specifici. Altre ricerche genomiche rivelano il coinvolgimento dei geni connessi all’attività dell’asse HPA, al Sistema Nervoso Simpatico e alla funzione immunitaria.

10. Esistono prove di uno stato di infezione attiva latente più frequente con diversi tipi di herpes virus e entero virus. Gli herpes virus comprendono il virus dell’Epstein Barr, l’HHV-6 e il citomegalovirus. Altri agenti infettivi, quali il batterio che causa la malattia di Lyme, il virus di Ross River e la Febbre Q, possono scatenare la CFS.

AIMS: The purpose of this study is to demonstrate that the use of
recombinant early antigens for detection of antibodies to
cytomegalovirus (HCMV) gene products CM2 (UL44, UL57) and p52 (UL44)
is specific in the diagnosis and differentiation of active HCMV
infection in a subset of patients with chronic fatigue syndrome (CFS)
which is often missed by the current ELISA assay that uses crude
viral lysate antigen.

METHODS: At a single clinic from 1999 - 2001 a total of 4,774
serologic tests were performed in 1135 CFS patients using two
immunoassays; Copalis immunoassay and ELISA immunoassay. The Copalis
immunoassay utilized HCMV Early gene products of UL44 and UL57
recombinant antigens for detection of HCMV IgM antibody, and viral
capsid antigen for detection of HCMV IgG antibody. The ELISA
immunoassay utilized viral crude lysate as antigen for detection of
both HCMV IgG and IgM.

RESULTS: Of the total, 1135 CFS patients, 517 patients (45.6%) were
positive for HCMV IgG by both HCMV IgG by both assays. Of these,
twelve CFS patients (2.2%) were positive for HCMV(V) IgM serum
antibody by HCMV ELISA assay, and 61 CFS patients (11.8%) were
positive for IgM HCMV serum antibody by Copalis assays. The Copalis
assay that uses HCMV early recombinant gene products CM2 (UL44, UL57)
and p52 (UL44) in comparison with ELISA was 98% specific.

CONCLUSIONS: Immunoassays that use Early Antigen recombinant HCMV CM2
and p52 are five times more sensitive than HCMV ELISA assay using
viral lysate and are specific in the detection and differentiation of
active HCMV infection in a subset of CFS patients.

Low grade inflammation and arterial wave reflection in patients with
chronic fatigue syndrome

Some of the symptoms reported by people with chronic fatigue syndrome
(CFS) are associated with various cardiovascular phenomena. Markers
of cardiovascular risk, including inflammation and oxidative stress,
have been demonstrated in some CFS patients but little is known about
the relationship of these and prognostic indicators of cardiovascular
risk in this patient group. We sought to investigate the relationship
between inflammation and oxidative stress and augmentation index, a
measure of arterial stiffness, in 41 well characterised CFS patients
and in 30 healthy subjects. The augmentation index, normalised for a
heart rate of 75 beats per minute (AIx@75), was significantly greater
in CFS patients than in control subjects (22.5 +/- 1.7 versus 13.3 +/-
2.3%, P=0.002). CFS patients also had significantly increased levels
of C-reactive protein (2.58 +/- 2.91 versus 1.07 +/- 2.16 g/mL,
P<0.01) and 8-iso-prostaglandin F 2alpha isoprostanes (470.7 +/-
250.9 versus 331.1 +/- 97.6 pg/mL, P<0.005). In CFS patients, AIx@75
significantly correlated with log C-reactive protein (r=0.507,
P=0.001), isoprostanes (r=0.366, P=0.026), oxidised LDL (r=0.333,
P=0.039) and systolic blood pressure (r=0.371, P=0.017). In a
stepwise multiple regression model, (including systolic and diastolic
blood pressure, body mass index, C-reactive protein, TNFalpha, IL-1,
oxidised LDL, HDL cholesterol levels, isoprostanes, age and gender),
AIx@75 was independently associated with log CRP (beta=0.385,
P=0.006), age (beta=0.363, P=0.022), and female gender (beta=0.302,
P=0.03) in CFS patients. The combination of increased arterial wave
reflection, inflammation and oxidative stress may result in an
increased risk of future cardiovascular events. Assessment of
arterial wave reflection might be useful for determining
cardiovascular risk in this patient group.
